Inflammatory Breast Cancer by Dr Kathleen Thompson

Have you read Heidi Loughlin’s blog? Heidi is a young mother who has breast cancer.

She was pregnant when diagnosed, so treatment was started with chemotherapy drugs which were relatively safe for her unborn baby. The birth was to be brought forward, when she would switch to more effective drugs.

However her cancer progressed quickly, requiring an earlier delivery than planned, to allow Heidi to start Herceptin treatment sooner. Devastatingly, her baby daughter died shortly after the birth, just before this Christmas.

Heidi has inflammatory breast cancer (IBC). Although rare  (1-4% of all breast cancers), it is important for two reasons— (1) it grows and spreads rapidly and (2) the appearance may mimic infection or injury (often there is no lump), and so breast cancer may be overlooked and life-saving treatment delayed.

IBC invades the breast skin, and blocks the lymph vessels within it. It tends to spread in sheets, rather than forming a lump.

This results in an unusual picture. The lymph blockages cause fluid build-up, so the skin swells and discolours, and hair follicles enlarge, forming multiple pits. The resulting skin resembles that of an orange (and is called ‘peau d’orange’).

However this classical appearance isn’t always seen either.

The breast may become red, hot and swollen, resembling infection, and if ‘mastitis’ doesn’t respond to antibiotics, IBC should be considered urgently.

Sometimes the skin appears bruised, or ridges or welts appear. There may be ulceration. Sometimes the nipple inverts or leaks fluid/blood, and there may be enlarged lymph nodes in the armpit or collar bone area.

If any of these changes occur, you must see a doctor without delay.

Because there may be no lump, and because it often occurs in younger women, with denser breast tissue, it may not be picked up on a mammogram.

Biopsy of the affected skin is key to diagnosis. AS IBC spreads early, PET or CAT scans of the body, and bone scans will also be needed.

The outlook for IBC used to be very poor. It is still not as good as for other breast cancers, but with modern treatment it has improved, and recent genetic research has yielded potential new hope.

Chemotherapy is usually started before surgery. Afterwards a full mastectomy is generally needed, removing more body tissue than normal, including armpit lymph nodes. Breast reconstruction isn’t normally possible until later on.

Radiotherapy follows surgery, then often more chemotherapy. If the cancer is HER2 positive, Herceptin will be given. If it is sensitive to female hormones, anti-oestrogens, either tamoxifen or an aromatase-inhibitor, such as letrozole will be prescribed.

Treating any cancer during pregnancy involves balancing risks to baby and mother, and, as in Heidi’s case, can involve bitter disappointments.

If you notice any unusual and persistent changes in your breast—not necessarily a lump, you must see your doctor without delay. Unless there is a good explanation for the changes, you should be referred to a breast clinic. Do push for this if you are concerned.

The Hateful Eight – Film Review

the_hateful_eightRolling in at a mammoth three hours and seven minutes, the aptly titled The Hateful Eight is a film that in many ways will divide audiences. Those that love Quentin Tarantino’s style of film-making; the long build ups, exaggerated sword plays of dialogue and immediate violence, the no expense spared approach to making scenes look and feel authentic will no doubt rejoice here. But for those who were perhaps looking for a quick fix of action seen in examples such as Django Unchained and the Kill Bill series are likely to be disappointed or lack the endurance to see this film through to the end. It follows then that this film can be seen as a blessing and a shame in equal measure.

Shot in Ultra Panavision 70 which is apparently super widescreen, a long, and I mean long intro follows a horse and carriage through what must be a the bleakest snow storm. Yes, it is almost ten minutes before we actually get any dialogue. Only Tarantino could do that and not break a sweat. But when people do start speaking its then that you appreciate the depth and texture that he places within his characters.

John Ruth (Kurt Russell) is a bounty hunter carrying his charge Daisy Domergue (Jennifer Jason Leigh) to a town called Red Rock. He never fails to bring his quarry in alive rather than dead so that they can be hanged by the state. Before long he is joined by Major Marquis Warren (played by an incredibly on form Samuel L Jackson) who after some convincing is permitted to share the stagecoach to Red Rock. They are soon after joined by Chris Mannix (Walton Goggins); allegedly Red Rock’s new sheriff, who makes it clear that in return for riding on the stage-coach he’ll ensure that justice will be done on arrival.

The worsening storm forces them to break their journey at Minnie’s; an isolated shop that they intend to use as a stop-over point and it’s here that four more faces join the crowd, potentially all eager for a share of the bounty that is on Daisy Domergue’s head. They are Sanford Smithers (Bruce Dern), an old Confederate general, Joe Gage (Michael Madsen), a taciturn cattle-hand, Oswaldo Mobray (Tim Roth), a flamboyant hangman, and Bob (Demián Bichir), the shop’s temporary Mexican caretaker. Minnie herself however is nowhere to be seen. This sets the scene for a set-up where no one can be trusted and anything can happen.

It’s here where the dialogue goes into overdrive and Tarantino really goes to work building and creating tension. However, it has to be said that it is a shame that it is such a slow build up; a growing crescendo which perhaps would have fared better if the characters had redeeming qualities. The reality is I found I could find little empathy with any one of the characters, they all had behaviours which I found distasteful, but perhaps that is the whole point. The second act plays out almost like an Agatha Christie novel; largely a quest to find out who is behind it all. The characters try and suss each other out. Flashbacks occur; who was where and when this or that happened and people try to talk their way out of being in the guilty frame.

What embellishes this is the fine cast. Very rarely do you find films where every actor shines. The only one I can think of right now where that occurred recently was in The Wolf of Wall Street and again it was because the director was very credible so the cast gave it their all. Here it is the same. They all implicitly trust Quentin so they hit their roles with vigour, the end result is stunning, at times riveting and in so many ways simply wonderful.

Personally, I am not a fan of long films. My friends were divided in equal measure between loving and hating this film. However, it has to be said from Tarantino this film seems intimate, like a personal project he really deliberated, stressed over, savoured and desired to share with his fans. The end result is something only he could realistically pull off at such quality and regardless of whether you love it or hate it you have to admit it’s a fine piece of work.

On last night’s red carpet, celebrities including Kate Hudson, Live Schrieber, Laverne Cox, Uzo Aduba, Eddie Redmayne, Amy Schumer, Gina Rodriguez, Jennifer Lawrence, Viola Davis, Sam Smith, Eva Longoria, Olivia Wilde, Jason Sudeikis, Jussie Smollett, Bryan Cranston, Olivia Palermo, Wiz Kalifa, Regina King and Instagram star Kristina Bazan joined Moët & Chandon for its beloved philanthropic initiative, “Toast for a Cause.”

A red carpet staple for many Hollywood A-listers, “Toast for a Cause” invited Golden Globes nominees and celebrities on the red carpet to raise a toast with Mini Moët for which Moët & Chandon donated $1,000 to the celebrity’s charity of choice in his/her name. As a result, Moët & Chandon donated to charities such as MDS for ALS, Art of Elysium, I have a Dream Foundation, Trans Tech, Place Called Home, Global Citizen and many more. New to the program this year, Moët & Chandon donated another $1,000 to a fund supporting aspiring young film makers via the inaugural Moët Moment Film Festival.

Moët & Chandon also served the Moët Ruby Red, a modern take on the French 75 and the official cocktail of the Golden Globe Awards.

The after-party saw celebrities sip champagne from Moët Minis delivered through a designed champagne vending machine.

Frost is delighted that Gillian Holmes of the editing house, The Editor, will, over the next few weeks, advise Frost’s readers on writing successful first chapters, and that tricky beast the synopsis.

 

The timing is particularly apt because the charity Words for the Wounded (sponsored by Frost Magazine) has launched its 2016 Independent Author Book Award  for self published books – fiction or non fiction/e-book or physical.  which is due to close on 6th March. Last year’s competition was won by Jane Cable, who as a result is now represented by Felicity Trew of the Caroline Sheldon Literary Agency.

It is Felicity who is judging the Words for the Wounded competition this year, so flex those fingers, sharpen your imaginations and let Frost, and Gillian, help you along the way.

 

Gillian Holmes  has been working in publishing for nearly twenty years. She started out at Quarto, working on craft books – applique and decoupage was very popular at the time and she’s endeavoured to forget everything she learned about those skills over the years.

 

She quickly moved on from that nightmare to work at Simon & Schuster where she worked with authors such as Jackie Collins, Ben Elton and Sean Hughes. As well as doing quite a lot of football, the X Files and books about UFOs.

 

Then on to Carlton, working on illustrated non-fiction and TV tie-in novelisations – the Coronation Street novel is a particularly proud moment. The entire 40 years in a novel.

 

But with three children, life in-house became difficult, so she worked as a freelance editor, copy editor and proofreader for various clients, before moving back in-house to Arrow Books six years ago, where she edited many bestselling authors, such as Katie Flynn, Dorothy Koomson, Amy Silver (aka Paula Hawkins), Cathy Woodman, Lisa Lynch and Margaret Graham, and many others.
Gillian is really looking forward, over the next few weeks, to helping those of you who are working on a novel, or perhaps to inspire those who are not, to do so.

Workers Feel ‘Trapped’ In Their Chosen Career By Age 36

Tips to get a hotel business off the groundNew Year is usually the time for change but could it already be too late? According to new research it is too late to change career by the time you reach 41.

A study found that most Brits want to be on their chosen career path by their mid-30s and there is no going back after turning 41.

The survey of 2,000 workers found people are most susceptible to feeling ‘trapped’ in their line of work at the age of 36.

Suzie Webb, AAT’s Director of Education at the Association of Accounting Technicians which carried out the study, said many people have plenty of time to change direction.

“This research shows that many people incorrectly believe that they have to stick with their career despite being a reasonably young age,” she said.

When probed further as to why they had doubts about retraining, 59 per cent of respondents said they feel they are trapped in their current career path or line of work, with the biggest reason for this being the feeling they can’t progress any further and are bored in their current role.

This led to 31 per cent saying they think about changing career around once or twice a month.

The fear of change may be what is holding people back. When asked to describe their current employment, workers were nearly twice as likely to use the word ‘convenient’ than they were to use words like ‘success’ or ‘fulfilling’.

Three in ten said they are stuck because they don’t know what else they could do, while nearly a quarter worry they wouldn’t be any good at anything else.

Well over a fifth said that it’s simply too risky for them to think about any other job.

One in seven said they committed to their career after having kids, while over a fifth confessed they felt too old to learn anything new.

In addition, one in six admitted they were hesitant to get re-qualified in a new area, because they were scared of being labelled ‘a mature student’ in classes.

Suzie Webb says: “Retraining is much less of a big step than people might imagine. There are so many options available – part time, evenings and via distance learning that offer you the chance to study around other priorities in life.

“Plus, people are working for considerably much longer than in the past – so it makes sense that our views towards retraining should also follow this pattern. Lifelong learning is the key to success.”

Taser Revelations was recorded at The Happiness Hotel, Song by Toad’s newly aquired warehouse recording space. Utilising the warehouse’s natural room reverb to great effect, the album features producer and multi-instrumentalist Robbie Lesiuk performing a variety of instruments including steel drums, organ and piano. Greggor Douglas plays the Roland Juno/omnichord and ex-Maple Leaves Anna Miles provides harmony vocals.

Continuing Stafford’s trajectory into electrified alternative pop, Taser Revelations begins with some of the most melodically catchy songs he has ever written, before taking a swerve down a dark alley midway through the album, venturing into industrial syncopation, samurai-guitar attacks and some unsettling, vivid imagery.
